community respite services

Are you looking for a safe space where your loved one with disabilities can enjoy supervised activities that are both fun and engaging? If so, our newest program might be right for you!

Starting in January 2016 Centerforce began offering Community Respite Services for adults with intellectual or developmental disabilities. Activities will be held at either our Lakewood Opportunity Center or out in the community. Care will be provided by trained Centerforce community advocacy staff. 

Services will be free for those who qualify, or otherwise offered at a low price.

To be eligible, clients must…

  • Be between 21 and 65 years of age
  • Eligible as part of DDA Medicare Waivers
  • Posses a copy of their individual care plan assessment
  • Complete a Centerforce intake
